It's because their billing agreements are made far before, and without consolation, the photographers and advertisement designers even get to working. Even if they did get to know the order of the names they would limitin themselves aesthetically to match them.

Here's an interesting one: Steve McQueen and Paul Newman both wanted first billing on The Towering Inferno. To reach an agreement McQueen was given "first" billing on the left but Newman was given "highest" billing. First time names were placed on different levels as so.
