I disagree. Some fantasies can never be acted upon, but they can still be fixated on to the same degree as attainable fetishes are. If you masturbate to the thought of getting your womb impaled by a unicorn every night for a year, I'm calling it a fetish even if it'll never happen.

Yes, it did. I think she felt weird and uncomfortable about it, so that's why she just briefly tried it and stopped. It stemmed from a brief conversation earlier that day, so she had it in her head and probably said "fuck it." Then felt bad or awkward.

I'd think of the fetish as the compelling need itself. Although that would suggest that the person involved would be compelled to pursue it one way or the other, there's still some degree of choice in the matter. Social pressures or ethical issues - or even just the practicality of the fetish - could step in to prevent it actually being realised, but wouldn't diminish the actual impulse the person is having.
