I don't think a fetish has to be involved in every single sexual encounter to be considered a fetish. I do think the inclusion of the object/action/whatever should either positively or negatively effect the success of the encounter (depending on whether it's included or not) to be a true fetish. And the effect doesn't have to be huge or anything - but if there were no appreciable difference then why have one (or many) at all?
