I tend to define fetish more narrowly than you guys; I don't think I'd include casual preferences. I'm not sure I agree with the dictionary in that the object of a fetish is 100% necessary to get off but I would say that for a true fetish there is "fixation to the extent that it may interfere with complete sexual expression" should the object not be involved.

I think that smoking or knee high socks CAN be fetishes. It depends on the extent of the attraction. If the person sees an attractive woman who happens to be smoking or wearing socks, and that enhances the attraction, then it's not necessarily a fetish. However, if the attraction primarily lies in the fact that the woman is smoking or wearing knee high socks, then that would count as a fetish for me.
