I would argue that back to back performances on two different stages is often closer to being truly back to back than having back to back performances on one stage, because of the downtime between acts at the same stage.
For example in 2008:
Cafe Tacvba (5:15 - 6:05) to Hot Chip (6:10 - 7:00) is more back to back than Portishead (9:15 - 10:15) to Prince (10:45) with half an hour in between.

Not going to claim it was the best ever, but My Morning Jacket into Roger Waters was pretty awesome.
Also liked Fleet Foxes into Band of Horses & YYY's into MBV
There is a lot of potential for sunday this year....Pavement into Thom Yorke...even LCD to Jay-Z could be cool.

My personal choice goes to Justice -> LCD Soundsytem in the Sahara in '07. Fucking incredible. Didn't really know much about Justice but the tent was totally insane, the we are your friends sing along was amazing and all in all just better than their '08 show. And LCD Soundsystem, wow, still one of the best shows I've ever seen. Got right front and center after Justice ended and the tent was absolutely wild. I'm excited to see them this year but no way it matches that show.
A very close to the aforementioned Kraftwerk -> Portishead but stupidly I was dragged away at the end of Kraftwerk to go see the MIA trainwreck so that kind of killed some momentum. And honestly I may get flamed for this but Prince was kind of a let down after Portishead. I had a good time but some of it was corny (the Sarah Mclachlan cover?!) and just couldn't measure up to the raw emotional power of Portishead. Dude is a killer showman though.

Since I'm having trouble thinking of this, it means I jump from stage to stage way too much.
But I'd argue that James Zabiela->Boys Noize->Erol Alken->Hot Chip on Sat afternoon in the Sahara in 08 was unbelievable!
Should there be any kind of honorable mention for that Sat stretch on the main stage in 06 too? Commom->Kanye->Sigur Ros->Franz Ferdinand->Depeche Mode... and then a walk across the polo to Daft?

There have been some pretty amazing b2b's (many mentioned already). I think my favorite would have to be the Kraft-Portis-Prince, but a close second is LCD-Rapture. The Aphex Twin-Dominos delivery in my hotel was a personal favorite, but there were only like 6 of us there for that one. There's a lot on here I wish I would've been there for (like NIN-Black Star and Bjork-Chemical Brothers).
This year I think Wolfgang-Proxy will take the cake for me and whatever I see for the last three hours of Sunday will be the close second.
